Football Liability Waiver and Release of Liability

 1. Acknowledgment of Risk 

I, the undersigned, acknowledge that participation in football involves inherent risks, including but not limited to injury, illness, or even death. I understand that these risks may arise from various factors, including my own actions, the actions of others, or the condition of the playing field. 

2. Assumption of Risk 

I voluntarily assume all risks associated with my participation in football activities organized by [Team/Organization Name], including but not limited to practices, games, and other events. 

3. Release of Liability 

In consideration of being allowed to participate in these activities, I hereby release, waive, and discharge [Team/Organization Name], its officers, employees, agents, and volunteers from any and all claims, liabilities, or causes of action arising from my participation in football, whether caused by negligence or otherwise. 

4. Medical Treatment 

In the event of an emergency, I give permission for [Team/Organization Name] to seek medical treatment on my behalf or my child’s behalf. I understand that I am responsible for any medical costs incurred. 

5. Governing Law 

This waiver shall be governed by and construed in accordance with the laws of [State]. 

6. Acknowledgment 

I have read this waiver, understand its contents, and agree to its terms. I understand that I am waiving my right to legal action in the event of injury.
